Qualifications

Job Information

Performs assigned duties, under the direction and direct supervision of the Pharmacist and in compliance with and as allowed by the California State Board of Pharmacy.

ESSENTIAL DUTIES
• Assists in gathering information and completing patient medication reconciliations in emergency department & acute nursing. This involves communicating with patients, pharmacies & providers.
• Coordinates with case manager, providers & pharmacist to provide ‘meds-to-beds’ before patients leave the facility or delivering them to patient’s homes.
• Perform patient medication centered calls.
• Assists pharmacist with the preparation, distribution, out-date monitoring, and accurate inventory control of medications throughout the facility.
• Supports and participates in hospital wide quality initiatives
• Adheres to regulatory and compliance standards

Qualifications

Minimum Education (or substitute experience) Required:
• High School or equivalent.
• Pharmacy Technician License with the State Board of Pharmacy
• BLS certification

Minimum Experience Required
• Three (3) years of experience in an outpatient or hospital pharmacy

Experience Preferred
• Knowledge of EMR system or ability to learn and use the district EMR.
• Hospital pharmacy experience
