Who Do We Provide Services To?

Rehabilitation Services provides care for inpatients, outpatients, and Skilled Nursing Facility (SNF) residents of all ages – from neonates to geriatrics.

Is a provider requisition/referral required?

Not necessarily. The Rehabilitation Services Department responds to both provider requisitions and self-referrals, as permitted under California’s Direct Access Law. A Physical Therapist may evaluate and treat a patient for up to 12 visits without a physician referral, after which a provider order is required to continue care.

Following an initial assessment, a treatment plan of care is developed and implemented with measurable goals. Progress is continually reassessed using evidence-based outcome measures to determine if further treatment is necessary.
