Case management department is here to provide our patients with a safe and appropriate discharge from the hospital’s inpatient units, Surgical services, and Emergency services. Our department is staffed with Registered Nurse Case Managers, Social worker, and a Patient navigator ready to assist you with home health services, home medical equipment, short term care and long-term care placement, and follow-up appointments with your primary care or specialty provider. If you find yourself in the ER, OR, or inpatient unit please contact the Case Manger if you have any questions or needs.
Our Patient Navigator is her to provide resources and support for non-medical circumstances and conditions that affect your health and healthcare decisions. These are the factors in your life and surroundings, like your income, where you live, your access to utilities, your job, and the community you’re a part of, that influence your well-being.
